Invoices are generated from charted work. This is the single most important design decision in billing: what was treated and what was billed come from the same record, so they cannot drift apart and no one has to reconcile them.
Payments can be split across methods and taken partially against a case. UPI, card and cash are recorded with the case they settle; the outstanding balance ages automatically from the invoice date.
Insurance claims move through submitted, queried and settled. An unsettled claim stays on the outstanding report, which is where most of the money a clinic has forgotten about turns out to be.
